MAGNIFIER CONTEST CONSENT FORM AND DIRECT NOTICE TO PARENT/GUARDIAN
Your child would like to enter the Magnifier kids’ art contest (the “Contest”), and we can’t wait to see the masterpiece they’ve created! Because this contest is just for kids, you (the parent or legal guardian, the “Parent/Guardian”) will submit the entry on your child’s behalf.
Before we go ahead, we need to make sure you are clearly informed about what information we will collect, how we will use it, and how you can control it.
A. COPPA NOTICE
1. Information we will collect if you submit this form
If you decide to complete and upload this form as part of your child’s entry, Magnificat (“Magnificat,” “we,” “our,” or “us”) will collect and use the following information in connection with your child’s participation in the Contest:
- Your child’s first name, age, and state or territory of residence;
- Your (Parent/Guardian’s) name and contact information (such as email address and mailing address);
- Your child’s or your general location information (city and state);
- Your persistent identifiers, such as cookies, IP address, or device identifiers, that are used to operate the Contest Site and ensure security;
- Photographs of your child’s artwork (the “Work”) and, if you choose to provide them, photographs of your child; and
- Any other information you voluntarily submit on the Contest entry form.
Providing this information is necessary for your child to take part in the Contest.
2. How we use this information
If you consent and submit this form along with your child’s entry, Magnificat may use the information listed above to:
- Administer and operate the Contest, including verifying eligibility and judging entries;
- Contact you about your child’s entry or potential prizes;
- Publish and promote the Contest, your child’s Work, and the Magazine, in accordance with the Official Rules and the consents you give below; and
- Maintain records consistent with Magnificat’s business needs and legal obligations.
- We may share this information with third-party service providers that help us operate the Contest, host websites, or perform other services on our behalf. These service providers may use the information only as necessary to provide those services and must handle it in a manner consistent with applicable law and our instructions.
Magnificat will not sell your child’s personal information for monetary consideration.
3. Your child’s participation and your rights
If you consent and submit this form, your child may participate in the Contest, and we will collect, use, and disclose your child’s personal information as described in this notice, the Official Rules, and our Privacy Policy.
If you do not consent or choose not to submit this form, we will not collect personal information from your child for this Contest, and your child will not be able to participate.
At any time, you may:
- Review the personal information we have collected about your child,
- Revoke your consent, and/or
- Request deletion of your child’s personal information.
- If you revoke your consent or ask us to delete your child’s personal information, your child will be disqualified from the Contest, and we will delete the personal information in accordance with applicable law.

4. How to provide your consent
To comply with children’s privacy laws such as the Children’s Online Privacy Protection Act (“COPPA”), we must obtain your verifiable parental consent before your child participates. Please:
- Read this notice and the Consent and Release sections below.
- Complete and sign the form. You may:
- Print, sign, and scan or photograph the signed form; or
- Upload the completed form as part of the online Contest entry, following the instructions on the Official Rules.

I authorize Magnificat to collect, use, and disclose my child’s personal information as described in this notice, the Official Rules, and the Privacy Policy, including:
- My child’s first name, age, and state or territory of residence;
- My name and contact information;
- My child’s or my general location information, as needed for Contest administration;
- Persistent identifiers (such as cookies and IP addresses) used to operate the Contest Site; and
- Photographs of my child’s artwork and, if I choose to provide them, photographs of my child.
- I understand that Magnificat may share this information with its service providers solely to operate and administer the Contest, host websites, and perform services on Magnificat’s behalf, and that Magnificat will not sell my child’s personal information for monetary consideration.
- I understand that if I do not provide consent, Magnificat will not collect personal information from my child for the Contest and my child will not be able to participate.
☐ I CONSENT to Magnificat’s collection, use, and disclosure of my child’s personal information as described above, and I authorize my child to participate in the Magnifier Kids’ Art Contest.
B. RELEASE FOR USE OF CHILD’S ARTWORK, IMAGE, AGE, AND LIKENESS
By checking the box below and signing this form, I, the Parent/Guardian:
- Represent and warrant that my child created the submitted artwork (the “Work”) and that it does not infringe any third party’s rights.
- Grant Magnificat and its affiliates a non-exclusive, worldwide, royalty-free license to use, reproduce, display, adapt, distribute, and publish the Work:
- On the Contest Site and Magnificat’s other websites and social media accounts;
- In print and digital editions of Magnifier magazine; and
- In editorial, promotional, and advertising materials related to the Contest and the Magazine.
- Authorize Magnificat to use my child’s: First name, age, and general location (city and state), together with the Work for the purposes described above.
- If I choose to provide photographs of my child, authorize Magnificat to use my child’s image and likeness in connection with the Contest and the Magazine as described above.
- Understand that this authorization is granted without additional compensation to me or my child, beyond any prize or recognition awarded in the Contest, and is subject to applicable law.
☐ I AUTHORIZE Magnificat to use my child’s artwork, first name, age, general location, and image/likeness as described in this Release.
C. ACKNOWLEDGMENT AND SIGNATURE
I understand that:
- I may, at any time, request access to the personal information Magnificat has about my child;
- I may revoke my consent and/or request deletion of my child’s personal information by contacting Magnificat at [email protected]; and
- If I revoke my consent or request deletion, my child will no longer participate in the Contest and Magnificat will delete personal information consistent with applicable law.
